Virtual Online Medical Assessments – Required Documentation
If you are unable to attend an in-person appointment and would like to enquire about the possibility of a virtual online medical, please get in touch to discuss whether this can be accommodated for your situation.
Should a virtual assessment be agreed upon, you will need to have the following information ready in advance:
Hospital Records
A copy of your hospital records relevant to your current medical condition(s)
List of Medications
A list of any current medications you are taking, including dosage and frequency
Scan Results
Any recent scan results or imaging (e.g. MRI, X-ray, ultrasound) that relate to your condition
Eyesight
Your most up-to-date eyesight and audiometry test results
Blood Pressure
Your most recent blood pressure readings
Having this information available ahead of the virtual appointment will help ensure a thorough and effective assessment.
